Our My Care Plan – Complex Care folder contains includes:

• My Personal Emergency Evacuation Plan (PEEP) – provides a concise summary of complex health information for consumers, carers and families needing to communicate with other providers especially during an emergency. It includes sections for Personal Details, Emergency Contacts, Healthcare Details, Contacts and Other Information.
• My Self-Management – self-management can take a bit of practice and lots of planning. Your healthcare provider or care team can help you get started by setting some targets and goals.  Learning about your condition and keeping track of your observations can help you self-manage and stay well. Your escalation plan can be personalised with your care team so that you know when you are on target and when you need to contact your GP or Call an Ambulance.
• My Medicines – helps you keeps track of allergies, alerts, action plans, devices, administration, and any changes to your medicines. Taking your medicines properly is an important part of self-management and keeps all the information in one place to keep your care team on the same page.
• My Care Team – there are lots of people involved in keeping you well. Your care team can include healthcare providers like your GP, Allied Health, and Specialists.  Don’t forget to include any carer and emergency contacts.
• My Health History – lets you keep track of any medical, surgical, or acute events which means you don’t have to wait for your electronic records to be viewed, updated, or transferred to other healthcare providers.
• My Family History – allows you record relevant family medical history including conditions that increase your risk for certain chronic conditions including cardiovascular disease, respiratory conditions, chronic kidney disease, diabetes, musculoskeletal conditions, and cancers. You can record information for your parents, brothers and sisters and any additional information to discuss with your healthcare provider.
• My Immunisations – is a record of relevant information including the date, vaccine, diseases protected against and any additional comments such as a reaction to a vaccine.
• My Health Goals – remember that all goals start with a vision. What are you wanting to achieve and why?  There are some planning tools you can use to help set some health goals on your own or with your health coach or care team.  Did you know that whether you succeed in achieving a goal can depend on how ready you are.  Write down a few health goals and then choose a health goal that you are “Super Ready” for. An annual plan can help you break down your goal into smaller goals and make you more likely to succeed.
• Monthly Plan Booklet – each monthly booklet has 8 pages and allows you to keep track of important dates and tasks, record observations, daily habit tracker and a monthly summary to review goals and achievements, end of month reflections and a summary of events which can be helpful for your healthcare team to review.
• A4 Folder – flexible folder with a clear pocket to hold additional paperwork.
• A4 Document Wallet – clear, expanding wallet that allows you to store additional papers, referrals, and results.
